Community Service Information
Please refer to the following document to get pre-approval and to submit community service hours for Bright Futures.
Hours should be pre-approved with your counselor using page 4.
Completed hours should be submitted using page 5 and 6. Page 6 may be replaced with a document from the agency used but must contain a signature of the supervisor in charge.
